Move & Update documentation

As of today, most of the adapter's documentation is detailed in the README file.
We want to move all documentation to https://clickhouse.com/docs/integrations/dbt, and leave the README file with repo-related instructions. 


Topics to add to existing documentation:
* Projections
* Recommendations and best practices for automating and using the adapter with different CI/CD systems.
