Update vk.xml

 - ./utils/update_vk_xml.sh <VER>
 - or manually
   - copy vk.xml from Vulkan-Docs to xmls/
   - copy headers from Vulkan-Headers to include/

Add an extension

 - add an extension string to VK_XML_EXTENSION_LIST
 - update VK_EXT_command_serialization.xml with the help of
   print_vk_command_types.py
