Improve the gitignore default (#58)

* rework gitignore completely

Thanks to the wonderful gitignore.io, I've gotten a lot of files to 
ignore in.  Thanks to Joe Blau `@joeblau` for his work on this website.

* Accept Manifest.toml and remove extraneous comments

* bump version
