As the title says, this PR tries to fix the "Invalid form" issue by creating a custom Hub image. This image is the same as jupyterhub/k8s-hub:4.2.0, except that it sets the
JUPYTERHUB_XSRF_ANONYMOUS_IP_CIDRSenvironment variable to try to prevent issues with XSRF cookies.
