- added IBIBLIO mirror (USA) - removed Indian mirror page as it is nowadays just an `iframe` of the original site - removed old commented-out server entries

Code blocks were not displayed when specifying the language for the block, so I removed that. I guess it's a highlighting issue. I also removed all the subsections for different regions as we do not have that many mirrors, and the page was looking too heavy with so many sections. I changed the level of some sections too.
