Migrate all Qiskit ecosystem projects Intersphinx to use the new docs website

Several of our docs projects use Intersphinx to cross-reference the Qiskit and Qiskit Runtime API docs. We should update the config to use the new docs website, like done in Qiskit/qiskit-ibm-runtime#2234.

One way to find projects is to look in GitHub search for intersphinx_mapping in the Qiskit, Qiskit-Extensions, and qiskit-community GitHub organizations.
