P-chain docs

Describe the bug
There are some P-chain docs in avalanche repo, which are not up-to-date, nor particularly visible. We should update them and them to a platformvm level README.

Expected behavior
Documentation should be up-to-date and linked to a platformvm package README

This issue has become stale because it has been open 60 days with no activity. Adding the lifecycle/frozen label will cause this issue to ignore lifecycle events.
