When the amount of RAM > 8GB, the recommended minimum swap size is 4GB, it's true even with RHEL 6.
And the 1.5x physical RAM swap size doesn't make sense on servers with hundreds of gigabytes of RAM.
We do not need to take hiberation into consideration especially in Ceph clusters I suppose.
