Output just a constant clipping with basic gyrator EQ circuit

As you can see in the screenshot below, I am trying to work on a basic EQ circuit (with the intention of expanding it out to multiple bands). I have gone over it many times, but can't seem to find any issues with the actual layout. For some reason, however, it just wants to blast the output with some kind of clipped signal. I am unable to get any audible version of my input (passive bass through an audio interface).

Any thoughts? It's greatly appreciated!

If you build a level pot after the output, does this resolve the issue? Anything above 1v = 0dbfs, so any signal above 1v will clip the output of the plugin, higher voltages are fine internally but the final signal has to be below that level.
