This would be an equivalent to effects like Disingenuous Dave or Jealous Jimmy that spawns Denise, in an ambulance, about to crash into the player, playing the line "I say become a doctor; you say become a patient!" She is hostile and attempts to repeatedly run into the player. If pulled out of her ambulance, she can attack with something suitable to her character, maybe a basic pistol or a melee weapon like a knife.

This would be an alternative to the typical "Seven Dollars" suggestion that would create something a bit more dynamic and interesting and funny, and less predictable than the over-meme'd option.

"I say become a doctor; you say become a patient!" is from a cutscene's audio, cutscene audio is the entire thing, not separated by dialogue line like in-game dialogues so can't be used unless the user choose to make the effect start audio that piece of audio from a mp3 file.

True, which shouldn't be too much of an issue since that's the case with a lot of different effects, including one of the ones I mentioned.
