Cannot edit/delete comments

In hypeapps_ui_get_entity_handler the comment and discussion_reply handlers are missing, and the comments JS has to account for the popup menu being outside the comment HTML (see Elgg/Elgg@14b7e3a).

I've fixed the comments on an item page, or in a listing below an object in the river, but for comment river items, there's no indication in the markup that it is a comment.

For some reason on my system comments are no longer associated with ElggComment, which has broken the comment edit page.
