Set phase = CONCLUDED when progressing into final state #13

Relates to hyperledger-labs/go-perun#147
A channel can be progressed into a final state. In this case, we may already set the phase to CONCLUDED. This would simplify the client logic as mentioned in hyperledger-labs/go-perun#147 .
