feat: Background auto-refresh for fallback keys #10
Description
Enable a background task to revalidate fallback keys automatically, as long as the system has available concurrency.
Acceptance Criteria:
- Only revalidates keys in fallback mode.
- Does not block main request flow.
- Logs each refresh attempt and result.
