Probably it could be achieved with code like streaming replication, and some other cronjob? based mechanisms to ensure full fidelity with the main database before being able to fully disconnect it and turn into a neon-only data source

Streaming (physical) replication can not be used in this case, because it requires that both database servers (Neon and your self hosted database) are identical. But you can use logical replication (publication and subscription mechanism) to perform such seamless migration.
