Threshold 0 issue

When scanning results in a file scoring 0, it seems that the tool will just throw, even if you set `minimumThreshold` to `0`.

Two issues here:

1. The bug above
2. The tool fails to tell the user which file was the worst (and triggered a threshold violation)
