Summary

Adds database schema for Shadow MCP access controls.

Includes:

  • shadow_mcp_approval_requests
  • shadow_mcp_access_rules
  • indexes and constraints for request/rule lookup
  • Atlas migration generated from schema changes

Testing

  • mise run lint:migrations
