Troubleshooting; mod not working, no toolbar available

I can't seem to get the mod to work, I don't see the toolbar option, and the toolbar mod doesn't have any real install instructions so I presume I drop it in the gamedata folder like other mods. toolbar as it appears

I cleared out my gamedatafolder and reinstalled all fresh.

I just started getting into the swing of things again, and I've been grabbing mods as I go; current list
